Envision Science Academy gladly welcomes volunteers in the school as they are paramount to our success!

To volunteer in classrooms, field trips or athletic events during the 2017-2018 school year, please complete the following:

Intent To Volunteer

The Intent to Volunteer is a form that captures your name and contact information into the 2017-2018 volunteer database. Once you enter your information into this form, ESA will begin to track the status of your background check  and other required forms. The Intent to Volunteer form can be found here.

On-line Volunteer Orientation

The Volunteer Training for ESA is now online! When you finish viewing all the slides, please fill out the form linked on the final slide.  You must view all the slides to fill out the form. To access the training, select the link found here.

Background Check

At Envision, the safety of our students and staff is of the utmost importance to us. As a security measure, background checks are required for all volunteers. Please submit the online background check form to our vendor, Carolina Investigative Research. The background check includes an investigation of statewide criminal records, driving records, Department of Corrections records, and the state and national sex offender registries. Envision has negotiated a price of $26 for first time ESA volunteers and $24 for returning ESA volunteers.  If you are charged a different amount, please contact the vendor. You will receive an email notification when your report is ready to be viewed online. Additionally, results will be reported to Envision Science Academy if you note your connection to the school in the comments section of your application. Once your background has been cleared, Envision will present you with volunteer options. Confidentiality

Our volunteers are required to maintain the strictest confidence with any information gained through interactions with students and staff at ESA. Volunteers may become aware of new information related to school operations and/or information about specific students (which may include such information as a student's academic performance, behavior, health, disabilities and related matters). Volunteers must keep this information confidential and not disclose or discuss it, except with school employees who have a need to know. Please print and complete the Confidentiality and Field Trip Driver Agreement and return it to the front office prior to your first volunteering experience

Liability Insurance

If you plan to be a field trip driver please bring a copy of your auto liability insurance upon attendance of one of the training sessions. The school needs the declaration page of your auto insurance that states the amount of liability insurance you have for your vehicle. You must have a minimum of $50,000 liability insurance. (This is North Carolina's minimum). This form can be obtained by calling your auto insurance company or going online through your auto insurance's website. This form is NOT your insurance card that shows proof of insurance.
