Envision Science Academy gladly welcomes volunteers in the school as they are paramount to our success! To volunteer in classrooms during the 2021-2022 school year, please complete the following:

- Background Check
- Review Volunteer Expectations
- Read and sign the Volunteer Confidentiality, Ethics, and Disclaimer Agreement
- Liability Insurance (*This is needed if you plan to be a driver for athletic events)
Intent To Volunteer
The Intent to Volunteer is a form that captures your name and contact information into the 2021-2022 volunteer database. Once you enter your information into this form, ESA will begin to track the status of your background check and other required forms. The Intent to Volunteer form can be found here.

At Envision, the safety of our students and staff is of the utmost importance to us. As a security measure, background checks are required for all volunteers. A background check must be done every year and is only valid for the school year. Please submit the online background check form to our vendor, Background Inquiries & Services found here. The background check includes an investigation of statewide criminal records, driving records, Department of Corrections records, and the state and national sex offender registries. Register online with Background Inquiries and Services, then enter “Envision Parent” as the organization (case sensitive) in order for ESA to access your background check. Our volunteers are required to maintain the strictest confidence with any information gained through interactions with students and staff at ESA. Volunteers may become aware of new information related to school operations and/or information about specific students (which may include such information as a student's academic performance, behavior, health, disabilities and related matters). Volunteers must keep this information confidential and not disclose or discuss it, except with school employees who have a need to know. Please print and complete the Volunteer Confidentiality, Ethics, and Disclaimer Agreement and return it to the front office prior to your first volunteering experience.
If you plan to be a driver for athletics please bring a copy of your auto liability insurance to the front office. The school needs the declaration page of your auto insurance that states the amount of liability insurance you have for your vehicle. Chaperones who drive must provide proof of liability insurance with a minimum of $50,000 per person for bodily injury. The declaration page can be obtained by calling your auto insurance company or going online through your auto insurance's website. The declaration page is NOT the same as your insurance card that shows proof of insurance.
