***All volunteers must go through the approval process each school year. Your approval expires before the first day of school each year***

Background Check

At Envision, the safety of our students and staff is of the utmost importance to us. As a security measure, background checks are required for all volunteers. A background check must be done every year and is only valid for the school year. Please submit the online background check form to our vendor, Background Inquiries & Services found here. The background check includes an investigation of statewide criminal records, driving records, Department of Corrections records, and the state and national sex offender registries. Register online with Background Inquiries and Services, then enter “Envision Parent” as the organization (case sensitive) in order for ESA to access your background check. If you have any questions or concerns regarding the background check, please call Background Inquiries & Services directly at 919-377-0917.

In order to DRIVE students for athletic events or field trips you will need to also complete the following steps:

Proof of Insurance

In order to drive students for any reason (such as field trips as athletic events), the school needs the declaration page of your auto insurance that states the amount of liability insurance you have for your vehicle. Chaperones who drive must provide proof of liability insurance with a minimum of $50,000 per person for bodily injury. The declaration page can be obtained by calling your auto insurance company or going online through your auto insurance's website. The declaration page is NOT the same as your insurance card that shows proof of insurance.
